Useful meeting notes help people see what was agreed and what still needs a decision. The distinction matters more than a polished summary.
Use this fictional training-session example. The notes and example output are authored for learning, not generated by live AI.
Source note 1: On 16 October 2026, Mira and Dev discussed the training session. Source note 2: Mira will prepare the agenda by 20 October 2026. Source note 3: Dev suggested inviting a guest speaker; no decision was made. Source note 4: The venue is not confirmed.
Find the commitment
“Mira will prepare the agenda by 20 October 2026” contains an action, an owner and a date. Those details can be carried into an action list without adding an assumption.

Keep the suggestion as a suggestion
Dev suggested inviting a guest speaker. The source also says no decision was made. “Dev will invite the speaker” would change the meaning. A safer entry is an open question: “Will a guest speaker be invited?” An owner and deadline for resolving that question are not specified.
The venue is also unconfirmed. Record the uncertainty directly. Do not choose a venue, infer a booking or invent a person responsible for arranging it.
Make the draft easy to check
Separate the summary, recorded actions and open questions. Keep references to the relevant source notes. A reviewer should be able to trace an action back to the words that support it.
When someone changes the draft, review the changed version again. If the original notes are corrected, keep the correction visible and check which parts of the output it affects. A previous review should not be treated as approval of new wording.
